Smart Forms V2 Overview

Smart Forms V2 is OutSmart’s drag-and-drop form builder. You start with a blank form, drag question types from a toolbox to add fields, and customize each one. When the form is ready, you send it to a patient by email or share a public link they can fill out without logging in.

This is different from Smart Forms V1, the older library-driven system where forms are built from pre-written question sections written by the OutSmart EMR team. Both versions are in the Smart Forms menu so you can choose which one fits a given form.

When to use Smart Forms V2

Smart Forms V2 is the right tool when you want to:

  • Build a form with your own wording, not from a pre-built library.
  • Add a mix of free text, multiple choice, signatures, matrices, and other question types in one form.
  • Share a single form with multiple practitioners in your clinic or across your linked offices.
  • Send forms by email to specific patients or publish a guest link patients can fill without logging into the patient portal.

How a Smart Form V2 reaches the patient

There are two ways to get a form to a patient:

  • Send directly to a patient. From the form list or the patient workspace, send the form to one specific patient. They get an email with a link they have 60 days to open. Once they open it, the 90-day fill clock starts.
  • Public guest link. Generate a shareable link from the Send panel. Anyone with the link can fill the form — they enter their name, email, and date of birth on the landing page first. Useful for clinic websites, QR codes, or posted intake links. Each time someone starts the form through the link, they have 90 days to finish. The link itself never expires.

In both cases the patient fills the form straight from the link — no Patient Portal account, no access code to type in. One click on the email link and the form opens.

What stays the same across V1 and V2

  • Both go through the same OutSmart email infrastructure with your clinic branding.
  • Both are linked to a patient record after submission.
  • Submitted forms from both versions appear together on the Smart Forms tab of the patient workspace.
