Editing A Smart Form V1
This guide covers the Smart Forms V1 system. For the newer drag-and-drop builder, see Smart Forms V2 in the main menu.
You might need to edit a Smart Form V1 Questionnaire at two different points in time: before you send it to any patients, or after it has already been sent to one or more patients.
Editing a Questionnaire Before It Has Been Sent to Patients
Editing a SmartForm Questionnaire is simple as long as it has not yet been sent to a patient.
- Go to the SmartForms page using the main navigation menu.
- Find the form you want to edit. In the right-most column for that form, click the Edit SmartForm button.
- This will take you to the SmartForm Builder page, where you can edit the content and configuration of your questionnaire.

If you see a notice that editing has been restricted, it means that someone has already started filling out your form. In this case, you will need to follow the instructions on how to edit a Smart Form after it has been sent to patients.
Editing a Questionnaire After It Has Been Sent to Patients
Once a Smart Form Questionnaire has been sent to at least one patient, it can no longer be directly modified. This is to avoid confusion for patients who might be in the process of filling out the form.
Instead, the only way to modify the questionnaire is to create a new version that you will use for all future patients.

- Go to the Smart Forms page using the main navigation menu.
- Find the form you want to edit. In the right-most column for that form, click the Duplicate Smart Form button to create a copy of your existing form.
- You can now edit and modify the new version of your form as needed. Once you are done, update any automated sending methods so that they now use this new form:
- Check Event Types where the form might be attached to a confirmation package
- Check your website in case you have provided a guest link to fill out the form
- Notify clinic staff and practitioners that the form has changed.
- After you have updated all the sending methods, you can archive the old questionnaire by clicking the Delete Smart Form button for that form. This will not delete any existing patient submissions of that questionnaire, but it will remove the form from the active forms list and prevent it from being sent to anyone else.
