Smart Forms V1 for Collaborative Environments

When you’re working in a collaborative environment, sharing SmartForms can help save time, promote consistency among practitioners and clinics, and give key staff access to critical information. The SmartForm sharing model allows practitioners to share patient questionnaire responses with others, even with those outside of their single clinic.

This can help to standardize questionnaires for research purposes and allow collaborative investigations by practitioners anywhere in the world.

How to Share SmartForms

SmartForm sharing settings are located within the SmartForm editing interface. There are a number of sharing options available:

  • Sharing a SmartForm with a practitioner: This lets a practitioner in your office, or in any other office, administer your SmartForm to their own patients. This option shares only the template, not patient submissions.
  • Sharing a SmartForm with an office: This allows you to share your SmartForm template with any practitioner currently part of the office you specify. If someone leaves that office, they will lose access to your SmartForm template.
  • Do you want to see all submissions of this form?: This option allows you, as the SmartForm template author, to see all patient submissions of this form, even if they are not your own patients. This is useful if you are conducting a research study and need access to all the results.
  • Do you want other practitioners to see all submissions?: This option lets you choose whether or not the practitioners you’ve shared this SmartForm with can also see all patient submissions, regardless of whether or not they are their own patients. This is useful if you are conducting a collaborative research project where all practitioners are sharing SmartForm submissions equally.
