Deleting a Chart

Our system never permanently deletes anything; all deleted charts are archived in the database and can be restored later. When deleting a chart, it is important to do so in a manner consistent with the legal requirements of your profession.

  • Deleting a Chart with Information: If the patient did not show up but you had already entered information, it is best to leave a quick note (e.g., “Patient did not show up, deleting chart”) before deleting it.
  • Deleting a Chart with No Information: If the chart was created by accident and contains no information, you can simply delete it without leaving a note.

You can delete a chart from only two places in OutSmart: From the Client Workspace, or from the Chart Editor.

Deleting a chart from the Chart Editor

You can open the Chart Editor by clicking the Continue Editing Chart button from a chart list (available on the SmartCharts page or Patient Workspace). Once in the editor, click the More Actions icon in the toolbar and select Delete Chart.

Deleting a chart can only be done from within the charting interface.

Deleting a chart from the Client Workspace

When you are in the client workspace, click on the “Charts” tab to see all of the patient’s charts. Next to each chart in the list will be a red trash icon. Click on this to delete the chart.

Restoring a deleted chart

If you deleted a chart by accident, you can restore it from the Client Workspace. Click on the “Charts” tab to see the list of charts for this patient. If there are any deleted charts, you will see a grey button above the list of charts that says “View Deleted Charts”. Click on this button to see your deleted charts. Deleted charts will be shown in red. You can then click on the circular arrow icon to restore the chart.
