Charting Basics

In OutSmart, a Medical Chart or SOAP Chart refers to the notes you take during a single encounter with a patient. All of a patient’s charts are stored in their Patient Workspace, which serves as their central folder in your clinic. It is important to remember that in OutSmart, a “chart” is for a single visit, not a patient’s entire history.

Creating a New Chart

A medical chart can be created from several different places within the OutSmart system:

From the SmartCharts Page

  1. Open the Main Navigation Menu and select SmartCharts.
  2. On the SmartCharts page, choose a patient from the drop-down list.
  3. Click the Create New Chart button.

From the Calendar

  1. Open the OutSmart Calendar and click on an existing patient event. This will open the Event Editor.
  2. Confirm the event type and patient are correct.
  3. Click the ‘clipboard’ icon at the bottom of the Event Editor to create a new chart.

From the Daily Schedule Card

The easiest and simplest way to create a chart is from the Daily Schedule Card on the Dashboard, which lists all of your appointments for the day. Simply click the icon next to the patient’s name to start a new chart.

From the Patient Workspace

If you prefer to review a client’s file and notes before starting a chart, you can do so from the Client Workspace. Once you are ready, you can create a new chart from here.
