Charting Reports

OutSmart has four Charting Reports that summarize what you’ve recommended or administered to patients across your SOAP charts:

All four are accessed from the Reports page in the Main Navigation Menu, under the Charting report-type dropdown. Charting Reports are visible only to practitioners — admin staff don’t see this category.


Scope and Access

Charting Reports always show your own patient chart data — the data is pulled from your SOAP charts only. There is no way to widen the report to another practitioner’s charts.

There is also no dispensary dropdown for Charting Reports. The reports cover all dispensaries you’ve recommended from. The Recommended Products Summary and Injection Summary surface the dispensary name on each row so you can tell which dispensary an item came from.


A detail listing of every product, prescription, and blend you’ve recommended in the chart’s Product section, within a date range. One row per item.

Filters

  • Start Date / End Date — the date range. Limits to SOAP charts created in this window.
  • Include Products — standard product recommendations with a dosage note. Checked by default.
  • Include Prescriptions — items with formal Rx details (dose, frequency, route, refills, etc.). Checked by default.
  • Include Blends — custom recipes and pre-blended products. Checked by default.
  • Exclude continued / discontinued recommendations — see the Carry-Forward Behaviour section below. Unchecked by default.

Columns

Each row shows the patient name, chart type and date, product name, type (Product, Prescription, or Blend), and the dosage or recipe details. The chart name links back to the source chart.

Sample — Prescriptions only, last quarter

A clean list of every prescription you wrote in the last three months, with no products or blends mixed in.

  1. Set Start Date to three months ago, End Date to today.
  2. Uncheck Include Products and Include Blends.
  3. Check Exclude continued / discontinued recommendations if you only want net-new prescriptions (most useful for prescribing-audit work).
  4. Click Submit.

An aggregated view — one row per unique product. Use this when you want to know which products you recommend most often, or when you first or last recommended a given item.

Filters

Same filter set as the Recommended Products Report: date range, Products / Prescriptions / Blends checkboxes, and Exclude continued / discontinued.

Columns

Each row shows the product name, type, dispensary it came from, the date the dispensary first stocked it, the all-time first and last recommended dates across all your charts, and the count of times you recommended it in the date range.

The all-time first and last dates are not limited to the date range — they show the full history. The date range only affects the count column.

Sample — Top recommendations of the year

Your most-used products for the year, in count order.

  1. Set Start Date to January 1 and End Date to December 31.
  2. Leave all three include checkboxes on.
  3. Check Exclude continued / discontinued recommendations so a product carried forward through six follow-ups counts as one recommendation, not seven.
  4. Click Submit. The results sort by count, descending.

Injection Report

A detail listing of every injection you’ve administered in the Injections section of your charts, within a date range. One row per injection.

Filters

  • Start Date / End Date — the date range.

Columns

Each row shows the patient, chart type and date, the injection recipe name, the ingredient breakdown (each ingredient with its concentration), and the dispensary the recipe came from.


Injection Summary

An aggregated view of injections — one row per unique recipe. Use this to see which recipes you administer most often.

Filters

  • Start Date / End Date — the date range. Affects the count column only.

Columns

Each row shows the recipe name, dispensary, the all-time first and last administered dates, and the count of times you administered it in the date range.


Carry-Forward Behaviour

When you create a follow-up chart for a patient, OutSmart automatically carries forward every product, prescription, and blend from the patient’s most recent active chart. Each carried-forward item lands in the new chart with the Continue previous recommendation option selected, so you don’t have to re-enter long-term medications.

This is helpful clinically but matters for reporting: a product that you recommended once and then carried forward through six follow-up charts shows up in the raw data as seven rows, not one.

The Exclude continued / discontinued recommendations checkbox controls how this is handled in the Recommended Products Report and Summary. When checked, the report skips any row where the practitioner left the carry-forward setting as Continue, Continue but remove schedule, or Discontinue. Only rows marked New / Refill / Update count.

The filter also applies to the first-recommended and last-recommended dates in the Summary report. With the filter on, “First Recommended” shows the date you actually wrote the recommendation, not a date on which it was merely carried forward from an earlier chart.
