Creating A Smart Form V1 Questionnaire

This guide covers the Smart Forms V1 system (the library-driven form builder). For the newer drag-and-drop builder, see Smart Forms V2 in the main menu.

Creating Your First Questionnaire

To create a new Smart Form Questionnaire, you can either start with a blank template or use one of our pre-created templates.

  1. Navigate to the Smart Forms page using the main navigation menu.
  2. Click the +New button in the top toolbar.
  3. In the pop-up dialog, you can choose a pre-designed template, such as the Adult Intake Template, or start from scratch with a Blank Template. For this example, select the Adult Intake Template and click Create.

This will take you to the Smart Form Builder page, where you can configure and customize your form.

On the Smart Form Builder page, you have several options to configure the form:

  • Form Name: Change the name of the form (e.g., “Adult Intake Form”).
  • Form Description: A simple description visible to your patients.
  • Form Instructions: Basic instructions for your patients (e.g., “Please fill out all sections of this form as completely as possible”).
  • Allow Patient To Submit Multiple Times: Set to “yes” for forms like a daily journal. For an intake form, set this to “no.”
  • Allow Other Practitioners To Have Access To This Form: This option allows you to share the form template you create with other practitioners. They can then use the form you created and send it to their patients to complete. This is useful if you want to have a standardized form for the entire clinic.
  • See All Submissions: If you’ve shared the form, you have a couple of options:
    • See All submissions of the form: This option allows you to see all submissions of the form even if the form was submitted by patients of another practitioner with whom you shared your form template. This is helpful if you work in a collaborative clinic and you want to see all submissions of the form because you coordinate patient care.
    • Allow other practitioners to see all submissions of the form: This allows other practitioners with whom you have shared this form template to see all submissions of this form, even those from patients of other practitioners at the clinic. This is helpful if you work in a collaborative clinic and patients may see other practitioners at the clinic, who may want access to the original intake form the patient submitted.

Customizing Your Questionnaire

Once your questionnaire is configured, you can add or remove sections and customize the questions within them. If you started with a template, many sections will be preloaded.

To add a new section, use the Select A Section dropdown list in the top toolbar. Choose a section to add it to the end of your questionnaire.

You can customize the questions by checking or unchecking the show box next to each question. Hidden questions will not appear on the patient-facing questionnaire. To remove an entire section, click the red icon at the top right of the section.

A Consent Section is added just like any other section. The Smart Forms system includes a Consent Form With Signature section, as well as several others without a signature box.

There are several consent sections you can add, and one consent section with signature.
We recommend that the consent with signature be the last section of the form.

The purpose of a consent section is to provide a patient with an option to agree to a specific consent text. Typically, the Consent Form With Signature is added as the last section, so the patient only has to sign once to acknowledge the entire form.

Changing the Order of Smart Form Sections

To change the order of sections, look for the Section Order field in the gray section configuration bar to the left of every section. Simply change the numbers to reflect the desired order, save the questionnaire, and reload the page.
