Questionnaires

The Questionnaires section embeds a Smart Form questionnaire directly inside a chart. You pick the questionnaire to attach, fill it in during the visit (with the patient or on their behalf), and the answers live alongside the rest of the chart.

This is useful for standardized assessments you administer in the visit rather than sending out ahead of time — PHQ-9, GAD-7, pain scales, treatment-outcome questionnaires — anything where the structured answers should land in this specific chart rather than as a standalone Smart Form submission.

Adding a Questionnaire Section

1. In the chart editor, click the + button on the page where the questionnaire belongs (commonly +O for an objective assessment or +A for an outcome measure).
2. Choose Questionnaires from the section-type menu.
3. A picker opens listing every Smart Form you’ve configured at the office. Pick the one you want to attach.
4. The questionnaire appears in the chart as a fillable form.

You can fill it in immediately or save the chart and come back to it. The chart’s auto-save covers questionnaire answers the same way it covers free-text fields.

Where Questionnaire Answers Show Up

• In the chart — inline, exactly as you entered them
• On the printed Treatment Plan — answers print with the chart (depending on the questionnaire and your print preferences)
• In the Patient Workspace Smart Forms tab — the questionnaire submission appears there too, alongside any Smart Forms the patient filled in themselves

Because the questionnaire is attached to a specific chart, the submission is automatically associated with both this visit’s record and the patient’s overall Smart Forms history.

Note

Embedding the questionnaire inside the chart keeps the answers part of the clinical record for that visit — they’re attached to the chart’s audit trail and timestamp. If you send the same questionnaire as a standalone Smart Form instead, the answers sit in the patient’s Smart Forms area but aren’t tied to a specific visit. For chart-anchored outcome measures, embed; for general intake, send.
