What is a Chart in OutSmart?

A Medical Chart, or SOAP Chart, in OutSmart refers to the notes you take during a single patient encounter. A new chart is created every time a patient comes to see you. All of these charts are stored in the Patient Workspace, which acts as a dedicated folder for that patient within your clinic.

While in some professions a “medical chart” may refer to a patient’s entire history, in OutSmart, a chart is specifically the record of a single visit.

Note

If you came from a system where “chart” meant the patient’s whole file, that’s the Patient Workspace in OutSmart — the folder that holds every chart for the patient. A single OutSmart chart is one encounter; the workspace is the patient’s complete record.
