Smart Forms V1 — Overview

This is the original Smart Forms system (V1). Forms are built from a curated library of question sections written by the OutSmart EMR team. For the newer drag-and-drop form builder where you compose questions yourself, see Smart Forms V2 in the main menu.

What are Smart Forms V1?

The OutSmart Smart Forms V1 module is a unique and powerful questionnaire system that allows you to gather information from patients using a provided library of questions.

Using Smart Forms means that a significant portion of your questionnaire work is already done for you. Our library contains hundreds of questions, organized into descriptive Sections that you can use in your questionnaires. For example, you can use our existing Smart Form Questionnaire Templates to automatically send detailed Intake Forms to your patients within minutes of setting up your account.

All questions in the Smart Forms module are added by the OutSmart EMR team. We moderate the questions individually to ensure a consistent experience for patients using the system.

How To Create A Smart Form

You can create a Smart Form from scratch by starting with a blank template and then adding various Sections to your questionnaire. These sections contain preset questions that you can hide or show as needed to customize your questionnaire. You can also use one of our existing templates as a starting point.

How To Edit A Smart Form

You may want to continue editing your Smart Form at a later date, or perhaps you want to change the content of your Smart Form after having already sent it to a few patients.

Sending Smart Forms To Clients

Smart Forms can be sent to and/or filled out by clients in several ways:

  • Via a link in an email.
  • Attached directly to a specific Patient Portal account.
  • Added to your website.
  • Attached to a specific event in your calendar, so they are automatically sent when a booking is made.
  • Filled out in the clinic on a tablet.

Viewing & Printing Submitted Smart Forms

You can view submitted Smart Forms from the Smart Forms page, the Patient Workspace, or from within a medical chart.
