Viewing Smart Form Submissions

After a patient submits a Smart Form V2, you can view their answers in two places: the patient’s workspace, and the Assignments tab on the main Smart Forms page. Both open the same compact answer view.


From the patient workspace

The fastest way when you already have the patient open.

  1. Open the patient’s record.
  2. Click the Smart Forms tab.
  3. Find the submitted form in the list.
  4. Click the eye icon to view the answers.

The Smart Forms tab lists both V1 and V2 forms for that patient together, with the newest at the top.


From the Assignments tab

The right place when you want to see every patient who’s been sent any form.

  1. Open the main menu > Smart Forms > SmartForms 2.0.
  2. Click the Assignments tab at the top.
  3. The table shows every patient who’s been sent a form, by you or by colleagues with whom you’ve shared a form.
  4. Find the row and click the eye icon to view answers (only appears once the patient has submitted).

The patient’s name in the Patient column is a link that opens their workspace.


What the answer view shows

The view is a clean question-and-answer list:

  • Each question appears with the patient’s answer below it.
  • Unanswered questions show No answer in grey italics.
  • Multiple-choice answers show the selected option’s text (not internal codes).
  • Matrix questions show as compact tables.
  • Signatures show as the signed image.
  • Multi-page forms show page titles as section headings.

There are no buttons to interact with — it’s a read-only view of what the patient submitted.


View the audit trail

The audit trail tells you what happened to a specific form assignment: when it was sent, when the patient first opened it, when they saved progress, when they submitted, any reminders that went out, any extensions you made (pre or post expiry), any extension the patient granted themselves through the deadline banner, and any changes you made to the patient or administering practitioner.

  • On the Assignments tab, click the list icon next to a row.
  • On the patient workspace’s Smart Forms tab, click the list icon on a row.

The audit trail opens in a panel showing each event with a timestamp in your office’s timezone. Useful when a patient or colleague asks “what happened with this form?” — the answer is right there.


Resend a reminder or give the patient more time

If a patient was sent a form but hasn’t filled it out yet, you have two actions beside their row:

  • Bell icon — sends a reminder email with the same link, no clock change. The patient gets a friendly reminder with a note about how many days they have left to submit. Use this for “they probably just forgot.”
  • Extend icon (🔄) — gives the patient more time. On a still-live assignment, pushes the current clock out so they have a fresh full window. On an already-expired assignment, issues a brand-new link (the old one stays dead) with the patient’s partial answers preserved. The bell is hidden on expired rows; the Extend icon is still available. Use this when time itself is the problem, not attention.
