How to redeem an access code

What is an Access Code?

An access code is a 4-word code that is provided by your practitioner. It gives you access to forms or medical records.

An Access Code is provided by your practitioner, either before your first visit (via e-mail, phone, or sms), or during your visit.

The purpose of an Access Code is to give you access to Questionnaires, Treatment Plans, Documents, Appointment history, etc.

For security reasons, is it strongly advised that you redeem your access codes as soon as you get them.  Leaving an access code lying around can potentially give an unauthorized person access to your information.  If you lose your access code, immediately contact your practitioner so that they can cancel it and issue you a new code.

Smart Form Access Links and Access Codes

A Smart Form Access Link is a link or button that is sent to you in an email by your practitioner.  Clicking on the link will give you access to the Smart Form that your practitioner wants you to fill out before your visit.  These e-mails may include a ‘backup Access Code’.  This code is only used if the Access Link does not work for you.

Smart Form Access Links and Access Codes will not give you access to your medical records – only a questionnaire.  Once you attend your appointment, you can ask your practitioner for a Medical Records Access Code, which you can use in the same way to unlock access to your medical data.

Medical Records Access Links and Access Codes

Important:  You must use Access Links and Access Codes immediately upon receiving them from your practitioner.  Delays in using the code can result in your medical data being compromised if someone else get a hold of the link or code. Access Codes for medical records are only valid for 5 days after having been issued. If your Medical Records Access Code has expired, you will need to obtain a new one from your practitioner.

A Medical Records Access Link is provided to you by email from your practitioner.  Clicking on the Access Link will grant you access to your medical records whether or not you have a Patient Portal Account.  Once you click on the link, you will be able to either create an account, or log into an existing one, and see your medical data.

If you want to gain access to your medical data and have been provided a printed Access Code by your practitioner, or if they have emailed you an Access Code instead of an Access Link, then you must manually create an account and apply the code, or log into an existing account and apply the code.  See below for information on how to manually redeem an access code.

Manually Redeeming Your Access Code

If you received an access code from your practitioner, or if you were provided an Access Link which is not working and have a backup Access Code that you would like to use, here is how you can use that Access Code to redeem access to your medical data or Smart Forms.

If you do not have a Patient Portal account

  1. Go to https://portal.outsmartemr.com
  2. Click on the “Register Here” link at the bottom of the page.
  3. Complete the whole form, all fields are required, and make sure to choose a password that is easy for you to remember but difficult for others to guess.

    This is what the Registration Form will look like. Make sure to enter the Access Code at the bottom, you will not be able to create a patient portal account without a valid access code.

  4. Enter the Access Code provided above into the Access Code fields on the registration form, and then click on “Submit”.
  5. You will be automatically logged in, and you will see your Dashboard. All your medical data will be accessible from here.

If you already have a Patient Portal account

  1. Go to https://portal.outsmartemr.com, and log into your account.
  2. Once you are logged into your Patient Portal account, click on the green “+Access Code” button at the top right of the interface.

  3. Enter the code provided on this page into the code fields that come up, then click on “Submit”.

  4. That’s it! Your dashboard will refresh, and you are now connected to your medical data.
