Editing a SmartForm Questionnaire

There are 2 different points in time that you may want to edit a SmartForm Questionnaire: Before you sent it to your first patient, or after it has already been sent to patients.

Editing A Questionnaire Before It Has Been Sent To Patients

Editing your SmartForm Questionnaire before you send it to patients is quite simple:

  1. Go to the SmartForms page using the Main Navigation Menu.
  2. From the main SmartForms page, look for the form you want to edit.  In the right-most column for that form, click on the   Edit SmartForm button to edit the SmartForm.  This will take you to the SmartForm Builder page, where you can edit the content and configuration of your SmartForm Questionnaire.
  3. If you see that the editing has been restricted, it probably means that someone has already started filling out your form.  Look at the section below on editing a questionnaire after it has been sent to a patient to edit your form.

Editing A Quesitonnaire After It Has Been Sent To Patients

After a SmartForm Questionniare has been sent to at least 1 patient, it can no longer be modified directly.  This would make it confusing for patients who are in the process of filling out the form.  Instead, the only way to modify the Questionnaire is to create a new version of the questionnaire that you will use from now on for all future patients.  You can then delete/archive the old questionniare.  This will not delete any of the existing patient submissions of that questionnaire:

  1. Go to the SmartForms page using the Main Navigation Menu.
  2. From the main SmartForms page, look for the form you want to edit.  In the right-most column for that form, click on the   Duplicate SmartForm button to create a copy of your existing form.
  3. You can now edit the new version of your form and modify it as needed.  Once you are done, update all the methods you are using to send the questionnaire to your patients so that they are receiving this new form.
  4. After you have updated all the methods used to send the questionnaire to the patient, you can then archive the old questionnaire by clicking on the  Delete SmartForm button for that form on the main SmartForms page.  Deleting a SmartForm simply archives it.  If you need to recovered a deleted questionnaire, click on the  View Archived SmartForms button in the SmartForms Top Toolbar.  This will display a list of archived SmartForms, and allow you to recover them.

 

You cannot edit SmartForms once they have been sent to a patient.  If you want to change anything, you must duplicate the SmartForm and make the changes in the new version.
