Your practitioner may have assigned you some forms to complete before your appointment date. It is important to complete this forms as early as possible, to ensure that you get the most value from your time with your practitioner.
These forms that the practitioner assigns you are called SmartForms, and they provide valuable insight for the practitioner into your chief concerns and your medical history.
There are 3 ways in which a practitioner may assign you SmartForms:
Here is what you would do in each situation:
When you book an appointment with your practitioner online, if they have indicated in our system that you are to fill out some forms for that appointment, those forms will be automatically assigned to you. After your online booking, you can simply go the forms immediately.
Your practitioner may choose to simply manually assign forms to you. In this case, you will receive an e-mail that contains an “Access Link” as well as a backup “Access Code” in case the link does not work for you.
Clicking on the Access Link will allow you to gain access to your forms in the patient portal, whether or not you already have an account. Clicking on the link will allow you to either log into an existing account, or create a new account.
You should not normally have to use the backup Access Code for any reason. However, if the link is not working for you, then you can see this article on how to redeem your Access Code.
In this case, your Booking Confirmation Email would have also included an “Access Link”.
